Primary Health Care
The nurse practitioner at Azalea Family Health Care is prepared to care for the whole family. Your child may come in for well child care, immunizations, any urgent or immediate needs. Men and women may keep up preventive health care needs by scheduling an annual physical. People in the childbearing years may need contraceptive care, advise on pregnancy planning, preventive measures such as immunization for HPV, or testing for sexually transmitted diseases. The aging population begins to have some needs to manage problems such as elevated blood pressure, cholesterol, menopausal, or prostate changes. Throughout the lifespan there are measures to take that will identify and treat early the onset of new problems.
Chronic Disease Management
Chronic diseases include respiratory problems such as asthma, chronic obstructive lung disease, emphysema, hypertension, hyperlipidemia, arthritis, and more. We are prepared to monitor lung function with in-office spirometry, watch for changes in the EKG, monitor laboratory values, and treat the conditions to meet the standards of care that are considered best practices at the time. Your provider is up to date with continuing education for all aspects of family practice. She has access to on-line resources, standards that are available on the Electronic Medical Record system, and actively participates in courses for maintaining her knowledge base.

Sports Physicals
Sports physicals are meant to be a thorough assessment of the cardiovascular, respiratory, musculoskeletal and neurological systems in order to identify the student at risk for problems if the body is stressed during exercise. Your child will have an opportunity to address the growth and development changes he/she is experiencing and learn about the measures to identify problems such as testicular cancer, breast changes. Your child will experience their most thorough evaluation at Azalea Family Health Care.

Referrals: Your providers at Azalea Family Health Care have a thorough knowledge of the resources in the community and in neighboring communities including Medford, Grants Pass, Coos Bay, Crescent City, Arcata, Eureka, Gold Beach, Eugene, Roseburg and Portland. When there is a need, you will promptly be referred.

Department of Transportation: Specific requirements are addressed for the person who will be driving commercially. The physical exam is an important measure to assure safety on the roadway and identify the driver with modifiable factors that can be addressed so that he/she can continue to have productive employment.
